New Client Intake Form This form is designed to help me get to know you better. You will be able to tell me about yourself and what your goals are for your health. Please print this form and bring it to our session together.
General Information and Office Policies I believe that having a clear understanding with my clients regarding general office policies will help us in working together. I have outlined some very important information on this sheet. Please read it carefully and let me know if you have any questions.
Business and Professions CodeThis informational sheet details California’s Business and Professions Code sections 2052, 2053.5 and 2053.6. These are the guidelines by which I am able to practice hands-on healing as a mode of complimentary self-care. Print this out for your records.
Release and Disclosure Form This form states that you have been given California’s Business and Professions Code, that you are aware of my scope of practice and have agreed to participate in healing sessions with me. Please print this out, read and sign it and bring a copy with you to our first session.
